Per FDA guidance on clinical decision support software (2022), tools that allow the clinician to independently review the basis of a recommendation are not regulated medical devices.
Different hospitals standardize on different frameworks based on their EHR, quality reporting requirements, and historical adoption. CMQCC is dominant in California and West Coast systems; AWHONN is the most common nurse-implemented tool; ACOG SMI aligns with physician-led bundles. Validation studies show all three identify similar high-risk populations.
