PPHrisk Compliance

Stop rebuilding hemorrhage compliance reports from chart audits, spreadsheets, and memory.

Built for L&D directors, nurse managers, quality officers, and maternal safety committees.

Daily PPH risk assessments become monthly Joint Commission, AIM, and CMQCC bundle-compliance reports — automatically. Plus audit logs, simulation drill tracking, QBL documentation, and MTP timelines.

PPHrisk Compliance

Your hemorrhage compliance report writes itself.

No more month-end chart audits. No more "wait, did we drill this quarter?" Every PPH risk assessment your team runs becomes a row in your Joint Commission, AIM, and CMQCC compliance report — assembled automatically and ready for the maternal safety committee.

⚠ Label syncs to your team feed if signed in. Use bed/room/case ID — do not enter patient name or MRN.

1 Prenatal / Baseline Factors

2 Intrapartum Factors

Decision support tool — not a diagnostic device. Clinical judgment supersedes this score. Risk computation runs entirely in your browser. When signed in, only de-identified data (bed/room label, framework, stage, risk factors, tier, notes) syncs to your team's encrypted database — no patient name, MRN, DOB, address, or other direct identifiers. See Privacy Policy.

Which framework should you use?

All three are evidence-based and widely accepted. Choose the one your unit has standardized — or compare across them.

CMQCC V3

California Maternal Quality Care Collaborative

The de facto US gold standard. Categorical Low/Med/High based on risk factors. Updated 2022 in the V3 Hemorrhage Toolkit.

  • Best for: Hospitals with maternal safety bundles
  • Style: Categorical (no numeric score)
  • Updated: 2022
AWHONN

Association of Women's Health, Obstetric and Neonatal Nurses

Numeric weighted score (0–17+). Most widely deployed in US L&D units. ACOG Safe Motherhood Initiative endorses it.

  • Best for: Nurse-led workflows; EHR integration
  • Style: Weighted numeric → 3-tier
  • Updated: 2022
ACOG SMI

ACOG Safe Motherhood Initiative

Three-tier framework aligned with AWHONN and CMQCC. Used in the ACOG Postpartum Hemorrhage Patient Safety Bundle.

  • Best for: ACOG-aligned units; physician-led workflows
  • Style: Categorical Low/Med/High
  • Updated: 2024 bundle revision

Frequently asked questions

Is patient data transmitted anywhere?

No. Every calculation runs entirely in your browser using JavaScript. No patient information leaves your device, and the site does not store any inputs. You can use it on the hospital wifi without HIPAA concerns.

Is this an FDA-cleared device?

No — this is a clinical decision support tool intended to assist licensed clinicians. It does not diagnose, treat, or replace clinical judgment. Per FDA guidance on clinical decision support software (2022), tools that allow the clinician to independently review the basis of a recommendation are not regulated medical devices.

Why three different scoring frameworks?

Different hospitals standardize on different frameworks based on their EHR, quality reporting requirements, and historical adoption. CMQCC is dominant in California and West Coast systems; AWHONN is the most common nurse-implemented tool; ACOG SMI aligns with physician-led bundles. Validation studies show all three identify similar high-risk populations.

What compliance frameworks does the report cover?

The monthly compliance report aligns with the AIM Obstetric Hemorrhage Bundle measure set (4 R's: Readiness, Recognition & Prevention, Response, Reporting & Systems Learning), the Joint Commission Perinatal Care standards (PC.02.02.05), and the CMQCC V3 Hemorrhage Toolkit quality metrics. It captures: risk-assessment completion rate, tier distribution by month, simulation drill participation, MTP activation timelines, and QBL documentation completeness.

What data is stored in the cloud vs locally?

PHI (patient names, MRN, DOB) never leaves the user's browser. Only de-identified risk-assessment results sync to the team feed: bed/room label, framework, stage, risk factors, tier, timestamp, and who entered it. Simulation drill logs and audit data are stored in encrypted Supabase tables gated by row-level security on team email domain. No BAA required for this de-identified pathway.

Can I integrate this into our EHR?

Yes — SMART-on-FHIR support is live. PPHrisk implements the SMART App Launch 2.0 framework with read-only patient scopes. When launched from Epic Hyperspace, Cerner PowerChart, or any FHIR-R4-compliant EHR, PPHrisk prefills risk factors from the patient's chart — Conditions (prior PPH, previa, accreta, chronic HTN, preeclampsia, fibroids), Observations (BMI, platelets, hematocrit, EFW), MedicationStatements (anticoagulation, magnesium), and Procedures (prior cesarean). The clinician confirms and adds intrapartum factors. Read-only — PPHrisk never writes back to the EHR. See SMART_DEPLOY.md for vendor registration steps. Contact us to bring SMART launch to your unit.

Who built this?

PPHrisk is a product of Epigenuity LLC, built for L&D clinicians who need a faster way to run PPH risk assessments and bundle compliance. Free for all clinicians, with no subscription, no usage limits, no fees.

Are the recommendations evidence-based?

Yes — recommendations are drawn from the CMQCC V3 Hemorrhage Toolkit (2022), ACOG Practice Bulletin 183 (Postpartum Hemorrhage), AWHONN Postpartum Hemorrhage Project, and the AIM (Alliance for Innovation on Maternal Health) Maternal Safety Bundle on Obstetric Hemorrhage.

About PPHrisk Compliance

Who this is for: L&D directors, charge nurses and nurse managers, hospital quality officers, perinatal patient-safety officers, and maternal safety committee chairs — anyone who has been asked, at the end of the month, to assemble a hemorrhage-bundle compliance summary from chart audits, spreadsheets, and someone's memory.

Postpartum hemorrhage is the leading preventable cause of maternal mortality in the United States, and the frameworks built to reduce it — AIM's 4 R's, CMQCC V3, Joint Commission Perinatal Care standards — all demand documentation: risk assessments completed, simulation drills logged, MTP timelines tracked, QBL recorded. The gap between the framework and the bedside isn't clinical knowledge. It's paperwork.

PPHrisk Compliance closes that gap. The same risk assessment a nurse runs at admission becomes a row in the monthly compliance report. The simulation drill the unit ran last Tuesday lands in the quarterly audit log. The MTP activation timeline gets timestamped in real time, not reconstructed from memory at the M&M conference. By the time the committee meets, the documentation is already done.

PPHrisk is free for all L&D units during pilot — built and maintained by Epigenuity LLC. Enrollment is open to additional units. Get in touch to join the pilot.
